Am I Eligible to enroll in the Student Health Insurance Plan?

Online Enrollment Periods

All International students with an F-1 or J-1 visa status (including ELI students) are required to enroll in the Student Health Insurance Plan. International and ELI students are automatically enrolled in the Student Health Plan and the premium will be billed to the student’s university account.

Students must actively attend classes for at least the first 31 days for which coverage is purchased. Home study, correspondence, and online courses do not fulfill the eligibility requirements that the student actively attend classes.

International and ELI students are automatically enrolled in the Student Health Plan. You can enroll your dependents during the below Open Enrollment Periods.

Claims

When you visit in network providers, they will file claims for you! Everytime you receive care and a claim is filed for services you received, you will receive an Explanation of Benefits, the summary of your processed health insurance claims, electronically.

If you don’t already have an account, visit myhealth.healthsmart.com to get started! Be sure to have your member ID card on hand! You can also download the app via Google Play or the App Store.

If you do not want to receive your EOB’s Electronically, you still have the option to receive paper EOB’s.  To continue receiving paper EOB’s, visit myhealth.healthsmart.com, click on “My Account” in the navigation bar, then select “manage”.  Select “No” for Go Green, and “Save Preferences.”

Referral Requirement

The student and Spouse should use the services of the Health Center first where treatment will be administered or referral issued. Expenses incurred for medical treatment rendered outside of the Student Health Center for which no prior approval or referral is obtained will be paid at the Out-of-Network level of benefits as specified in the Schedule of Benefits. A referral issued by the SHC must accompany the claim when submitted. Only one referral is required for each Injury or Sickness per Policy Year.
A Bill and Lucille Magers Family Health and Wellness Center referral for outside care is not necessary only under any of the following conditions:
1. Medical Emergency. The student must return to Bill and Lucille Magers Family Health and Wellness Center for necessary follow-up care.
2. When the Bill and Lucille Magers Family Health and Wellness Center is closed.
3. When service is rendered at another facility during break or vacation periods.
4. Medical care received when the student is more than 50 miles from campus.
5. Medical care obtained when a student is no longer able to use the Bill and Lucille Magers Family Health and Wellness Center due to a change in student status.
6. Maternity, obstetrical and gynecological care.
7. Mental Illness treatment and Substance Use Disorder treatment.

Dependent children under the age of 6 are not eligible to use the Bill and Lucille Magers Family Health and Wellness Center, and therefore, are exempt from the above limitations and requirements.
